SEND-focused programme to develop primary–secondary transition. Working collaboratively with primary and secondary schools to support smoother transitions through initiatives that include:
- Standardisation of data sharing between primaries and secondaries
- Transition Process: developing a minimum standard for the transition process especially for those with SEND, including secondary transition events being held on the same weeks.
- Parent/carer support: focus on supporting parents and carers through the transition process.
- Develop a Transition Network system: secondary and primary feeders networks, secondary transition leads and Transition Advisory Group (specialists in transition to ensure schools are informed about the latest transition initiatives).
